Young Eagle Request Form

This is a preliminary registration only to determine interest for the courses. While we will try to make sure everyone has a chance, it does not guarantee your child will receive a Young Eagle Flight. Note safety is our first concern so due to weatheror any other reason this event may be cancelled with very short notice.

Enter the child's first name (must be between 8 and 17 year old). If more than one related child is flying please enter all their names.	(* Required)

Has the parent or legal guardian read the official registration form and legal waver? (Yes or No)	(Yes or No Required)
What time do you plan to be at the airport?	(Enter a time between 9AM and Noon)
